Regulations of the Physiotherapy Practice
I. GENERAL PROVISIONS
1. The professional practice: “Fizjoterapia mgr Maciej Gruba” hereinafter referred to as “practice” operates on the basis of:
- the provisions of the Act of April 15, 2011 on medical activity,
- entry no. 000000283972 in the register of entities performing medical activity maintained by the National Council of Physiotherapists,
- these Organizational Regulations.
2. The main location for providing health services is in Rzeszów at Beskidzka Street 95/10.
3. Other locations for providing health services: possible at the place of request upon individual arrangement.
II. PURPOSE, TASKS, AND SCOPE OF SERVICES PROVIDED
1. The purpose of the physiotherapy practice is to restore, improve, or maintain the health of patients.
2. The task of the practice is to provide health services in the field of physiotherapy, particularly:
- physiotherapy visits, including at home,
- functional assessment and physiotherapy,
- physioprophylaxis,
- issuing opinions, certificates, and statements.
III. TYPE OF MEDICAL ACTIVITIES
1. The practice will provide outpatient health services encompassing services in the field of physiotherapy.
2. Services will be provided in the practice premises and/or at the place of request.
IV. PROCESS OF PROVIDING HEALTH SERVICES
Patients are registered and accepted in the order of telephone/personal/internet requests. The service provider reserves the right to cancel a visit due to emergencies.
V. FEES FOR PROVIDING MEDICAL DOCUMENTATION
A copy of the medical documentation is issued without additional charges.
VI. ORGANIZATION OF THE PROCESS OF PROVIDING HEALTH SERVICES IN CASE OF CHARGING FEES, AMOUNT OF FEES FOR HEALTH SERVICES
1. For health services provided that are not financed from public funds, fees are charged to Patients.
2. The amount of fees for health services other than those financed from public funds is specified in the annex to the organizational regulations.
VII. FINAL PROVISIONS
1. In matters not regulated by these regulations, the provisions of the Act of April 15, 2011 on medical activity shall apply.
2. The regulations come into effect on the date of entry of the practice into the register of entities performing medical activity maintained by the National Council of Physiotherapists.

Price List of Physiotherapy Services:
- Individual physiotherapy - 170 PLN
- Diagnostic visit with gait analysis - 300 PLN
- Individual physiotherapy for mountain lovers - 170 PLN
- Physiotherapy package - 450 PLN
- Small package for technique repair - 550 PLN
- Large package for technique repair - 800 PLN
- Package for injured runners - 840 PLN
- Kinesiology tape package - 50 PLN
- Kinesiotaping - 30 PLN
- Visit at the place of request 170 PLN + travel costs (the service provider reserves the right to refuse a visit at the place of request for logistical reasons)
Packages and vouchers are non-expiring and non-refundable.
